What is an ACP‑5 form?
- ACP‑5 is the Asbestos Assessment Report required by NYC DOB and filed through DEP’s Asbestos Reporting & Tracking System (ARTS) for many construction, renovation, and demolition projects.
- It is completed, signed, and sealed by a DEP‑certified asbestos investigator after an asbestos survey of the areas affected by the proposed work.
- The form certifies that either there is no asbestos‑containing material (ACM) in the work area, ACM will not be disturbed, or any ACM to be abated is within “minor project” thresholds (under 10 sq ft or 25 linear ft).
- DOB relies on a valid ACP‑5 (especially with specific boxes checked, such as 8d for full demo) to issue many work and demolition permits in NYC.
